Welcome to the famous doctor's hall. Director Lu, we often hear that stars, experts and scholars committed suicide because of depression. Some people around us often say, "I am very depressed today?"? What exactly is depression?
Lu Ya: There is a saying that the 21st century is the era of depression, depression is a mood disorder, and the level of depression for a long time is depression. It created a long period of depression. Clinically, the patient is not happy, depressed, listless, sad or even wants to commit suicide. In severe cases, depression and stagnation will occur, accompanied by hallucinations and delusions.
If you had manic episodes during or before the onset of depression, you should diagnose bipolar disorder rather than depression. At present, it is the fourth largest disease of our mankind. The World Health Organization predicts that in 2020, it may become the second largest disease of human beings after heart disease.
Host: You just said that depression is now the fourth largest disease in the world. How about its incidence in our country?
Lu Ya: Different regions have different research methods and use different diagnostic standards. Regional differences are relatively large. In general, the survey of incidence in China is far lower than that in foreign countries. The survey of Beijing in 2003 was about 8% for women and 5.01% for men. By 2007, the overall terminal prevalence rate was 6%. If all depression disorders, except unipolar depression, including bipolar depression, including depression caused by physical diseases, and depression caused by psychological factors are added together, the lifetime prevalence rate is still high. 5% - 12% for men and 10% - 25% for women. The incidence of the disease is about twice that of men and women.
Host: According to the survey, the incidence rate of women is significantly higher than that of men. The main reason is that the incidence of disease is higher due to physiological reasons. What are the common types of depression?
Lu Ya: Depression is common. There is a chronic and mild depression called dysthymia. There is also depression, and there are other causes of depression.
For example, depression caused by physical diseases and psychosocial factors lead to such depression. And post schizophrenia depression.
Clinically, we divide depression into mild, moderate and severe levels to see whether it is accompanied by psychotic symptoms.
Host: There are many types of depression. Which of these types mentioned just now has the highest incidence?
Lu Ya: The incidence of severe depression is the highest.
Compere: Is the rate of medical treatment high?
Lu Ya: The rate of seeing a doctor is very low. At present, 60% of patients do not see a doctor. I haven't seen any hospitals, and 30% of people go to general hospitals. Because he has a variety of discomfort, such as panic to see the cardiology department, I insomnia to see God.
Keep looking at departments such as TCM, neurology and internal medicine. Only about 5.8% of the patients actually went to specialized hospitals for medical treatment.
Compere: Its medical treatment rate is so low. Is it a lack of awareness or a lack of knowledge about depression?
Lu Ya: It has been investigated that in the early stage of depression, less than 10% of patients can realize that they may suffer from depression. More than 70% of patients feel that I don't know what my current state is. 10% of the patients think I am a mental problem and I am not sick at all.
Host: It is mainly the lack of knowledge.
Lu Ya: Yes.
Host: I heard a word called "depressive disorder". What is depressive disorder?
Luya: Depressive disorder usually refers to depression.
Host: It's another name for depression. What are the specific causes of depression?
Lu Ya: The specific reasons are not fully understood. They are probably biological factors and social psychological factors. The result of the joint effect of the two factors depends on their individual differences.
Compere: It's still different from person to person. Do depression and anxiety often coexist?
Luya: It often exists at the same time. More than 60% of patients with depression will be accompanied by anxiety.
Host: Is it related to schizophrenia?
Luya: Depression and schizophrenia are two different diseases. Depression can be accompanied by symptoms of schizophrenia, and hallucinations and delusions can occur. But it changes with the change of mood. If the emotional symptoms disappear, the mood improves, and the symptoms will disappear.
Host: Speaking of depression, many friends were shocked when they heard that they had depression. I don't think I can help it, is that so?
Luya: No. Because now, depression is a common mood disorder with a high incidence of disability. If early treatment can be found in time, it can be controlled.
Compere: What harm and impact does it have on people?
Luya: If the treatment is not reasonable, the recurrence rate is very high. 10% of patients become chronic, and 15% of patients will commit suicide. As people become very depressed, their non-specific immunity declines and they are more likely to suffer from other diseases.
Compere: Is it serious enough to be fatal?
Lu Ya: Yes.
Host: Who is prone to depression?
Lu Ya: The main population has a positive family history. Depression is not a pure genetic disease but has a genetic tendency. More than half of patients with depression have a positive family history. If relatives and friends have this disease, they will be more dangerous and more likely to get sick. This is a conclusion.
Other people, including those with bad life experiences in childhood. Also, life pressure is particularly high, and the living environment is particularly bad. There are also elderly people and many patients with chronic physical diseases. And just said that the incidence of women is relatively high, for example, people who stay at home every day are more likely to get sick.
Host: House men and women are also high risk groups of depression. People with chronic diseases also have a high incidence of depression, including which diseases?
Luya: Patients with myocardial infarction, hypertension and cerebrovascular disease are more likely to suffer from depression due to cerebral hemorrhage and dementia. Patients with diabetes are more likely to suffer from depression. Patients with nervous system diseases and epilepsy are at great risk of depression.
There are also patients with malignant tumors, as well as AIDS patients, who are at a higher risk of getting sick.
