Why do children have femoral head necrosis?

Ms. Zhang called to ask: My child is 4 years old this year. Recently, he has always said that his right leg hurts. He dare not use his right leg when he walks. After going to the hospital for examination, the doctor considered that the child's femoral head was necrotic, and the child's right leg was not allowed to walk with weight, and he should go to the hospital regularly for reexamination. Excuse me, how can children have femoral head necrosis?

Dong Shuping, attending physician in the outpatient and emergency department of Shenzhen Pingle Orthopedic and Traumatological Hospital, replied: Children's femoral head necrosis, also known as epiphyseal chondrositis or flat hip, mostly occurs in children aged 2 to 12 years.

At the beginning of the disease, it will be difficult to walk or limp, and the affected hip can not be flexed, extended or adducted. At this time, parents of children often mistakenly think that it is sprain and do not pay attention to it, thus delaying the course of disease, causing the femoral head to flatten the "flat hip", resulting in lifelong disability. Early diagnosis and treatment are very important.

The study found that the main cause of avascular necrosis of the femoral head in children was trauma. Trauma is the first to damage soft tissue. If not treated in time, it can cause inflammation around the hip joint. Inflammation continuously compresses the joint cavity, forming a high pressure area in the cavity, which compresses the artery vessels supplying the femoral head, affecting the blood supply of the femoral head and causing necrosis.

In addition, children's carelessness in playing can also easily cause dislocation of the hip joint, contusion of the femoral head, and slippage of the epiphysis. These injuries can cause vascular damage around the femoral head in children. After the blood vessels were injured, part of the femoral head epiphysis lost blood supply, resulting in avascular necrosis of the femoral head epiphysis.

The onset characteristics of avascular necrosis of the femoral head in children's epiphysis are that most cases have no pain in the early stage, hip joint movement dysfunction, and the onset is concealed. Therefore, necrosis of the femoral head in early children is not easy to find, and it can only be found when children have claudication.

Environmental factors are also one of the influencing factors, including perinatal and postnatal living conditions. For example, the incidence of breech delivery children is four times higher than that of normal children, as well as parents' older childbearing age and family financial difficulties.
