"Rehabilitation is to wipe away the haze in the heart of patients." This is the perception of Xiao Juan, a rehabilitation physician at the Tiantan Xiaotangshan Rehabilitation Center of Beijing Xiaotangshan Hospital.

Xiao Juan has devoted nearly 9 years to rehabilitation work since she started working as a master student. Over the years, she has taken turns in PT (physical therapy), OT (occupational therapy), ST (speech therapy), physiotherapy and other rehabilitation treatment rooms, and has done a lot of rehabilitation treatment for patients.

What is rehabilitation? What is the meaning of rehabilitation? Xiao Juan has always been thinking about these issues in her work. In the past, people thought that rehabilitation was just care and care, without any technical content, but now they gradually realize that rehabilitation is a science. In fact, good rehabilitation can significantly reduce the sequelae, greatly reduce the pain of patients, and make their lives more quality and dignity. This is Xiao Juan's opinion, and is also the consensus of the medical staff of Beijing Xiaotangshan Hospital Tiantan Xiaotangshan Rehabilitation Center.

On September 1, 2021, Beijing Tiantan Hospital and Beijing Xiaotangshan Hospital jointly set up the Tiantan Xiaotangshan Rehabilitation Center (referred to as "Tianshan Rehabilitation Center") in Beijing Xiaotangshan Hospital, which is a high-level and high-level rehabilitation medical platform featuring neural rehabilitation and integrating rehabilitation medicine, teaching, scientific research, transformation and training.

"Medical humanities should be fully implemented in the whole medical activity. Especially in the process of rehabilitation, how to respect patients, how to care for patients, how to pay maximum attention to patients is a culture, and this culture can not be achieved by one person, nor can it be done overnight." said Jiang Yue, president of Beijing Xiaotangshan Hospital, "Most of the patients received by Xiaotangshan Hospital, especially the Tianshan Rehabilitation Center, are patients with neurological rehabilitation, and most of them have limb dysfunction. We put humanistic care throughout the whole medical practice process, and through a series of measures to improve their functions to the greatest extent, the ultimate goal is to enable these patients to return to their families, return to life, and return to work."

Rehabilitation patients especially need humanistic care

Qin Feng (not his real name) was over 60 years old and came to Tianshan Rehabilitation Center because of cerebral infarction for 2 weeks and left hemiplegia. "According to her wife, Aunt Qin has a sensitive personality. After her illness, she was greatly stimulated. She washed her face with tears every day. She can't accept the current state, can't sit steadily, and can't walk. She feels that she is a burden and burden at home. She is unwilling to come for rehabilitation treatment and take medicine. She feels that there is no sense in living." Xiao Juan recalls an unforgettable thing for her.

As the doctor in charge, Xiao Juan did not want to see Qin Feng continue to be depressed. In addition to arranging regular rehabilitation treatment for her, she also arranged a psychotherapist for psychological treatment and assisted with medication.

"In the first two weeks of Aunt Qin's arrival, I would visit her several times every day, explaining to her the cause, treatment and prevention of cerebral infarction in detail each time, as well as how to carry out rehabilitation treatment to achieve the desired effect, so as to enhance her confidence in overcoming the disease." Xiao Juan said that after several days of rehabilitation training, Qin Feng was gradually able to sit still.

Seeing his progress, Qin Feng's mood gradually improved. The number of sad tears became less and less, and the rehabilitation treatment was better and better. "Aunt Qin told me that there are so many teachers to help her with rehabilitation treatment every day, and a psychologist to chat with her. She felt everyone's attention and concern, and felt that there was no reason to continue to be depressed. She should train well, not let her family worry, and not live up to our expectations of her." Xiao Juan said.

After a month of rehabilitation treatment, Qin Feng was able to stand alone and walk a few steps with the help of others, making significant progress. After another two months of treatment, she can walk more than 100 meters by herself. "In the days to come, my aunt will smile when she sees me, and her life will gradually return to normal when she no longer frowns." Xiao Juan said, "The core of rehabilitation treatment is active movement. Passive movement rehabilitation can maintain joint mobility, prevent deep vein thrombosis, prevent cardiopulmonary function and other complications, but if the dysfunction is to be further improved, the patient's subjective initiative must be mobilized."

Tianshan Rehabilitation Center mainly treats patients with motor, cognitive, speech and swallowing dysfunction caused by central nervous system diseases such as cerebral infarction, cerebral hemorrhage, brain trauma, and brain tumor surgery, as well as various degenerative and immune diseases of the nervous system such as Parkinson's disease, Guillain Barre syndrome, multiple sclerosis, myelitis, multiple system atrophy, dementia, and knee Rehabilitation treatment of orthopedic diseases such as hip replacement and cervical and lumbar diseases.

"Rehabilitation patients especially need humanistic care." Kang Xiaoping, vice president of Beijing Xiaotangshan Hospital, said, "Because many patients with cerebral infarction or stroke still go to work normally the day before the onset, and the next day they have hemiplegia, the psychological impact is particularly great, and the impact on the family is also particularly great. Therefore, we take patients as the center, constantly improve humanistic services, help them out of the haze psychologically, and actively recover. "

Rehabilitation is part of treatment

"On the left wing of the capital, Xiaotangshan is a colorless hot spring. Thousands of new buildings have been built, and the Jade Pool is clear. Spring is doubted in winter, the new wind is warm, and turtles swim in the stream. What is Huaqing like? It's hard to say..." Guo Moruo described the winter beauty of Xiaotangshan in Niannujiao Xiaotangshan. The reporter got the same feeling when walking in Xiaotangshan Hospital a few days ago.

Xiaotangshan Hospital covers an area of more than 600 mu, and 1/3 of the whole hospital area is designed in Suzhou garden style. The small bridge is elegant, the pond is crystal clear, the trees form a forest, and the pavilions and towers are scattered in it. It is a beautiful place, so it became a hot spring palace in the Ming and Qing dynasties.

"The environment of Xiaotangshan Hospital has different beauty in spring, summer, autumn and winter. Rehabilitation patients will get physical and mental pleasure when they are in it. For this, we have combined the beautiful environment with the rehabilitation work to the greatest extent," said Kang Xiaoping.

It is a Tianshan rehabilitation center surrounded by beautiful scenery and soup springs. The total area of the treatment room is more than 1000 square meters, with rehabilitation treatment and training centers such as PT, OT, ST, physiotherapy, traditional Chinese medicine, and hydrotherapy. Zang Dawei, director of the Rehabilitation Department of Beijing Tiantan Hospital, serves as the center director.

According to Kang Xiaoping, the two hospitals formed a Tianshan rehabilitation team in the mode of "two hospitals and one department", bringing together more than 80 outstanding talents in neurological rehabilitation and related disciplines. It is a professional rehabilitation technical team with excellent technical ability, high educational level and reasonable echelon setting.

"Rehabilitation is a part of treatment. For diseases, prevention is the first step. If the disease breaks out, the acute stage is usually one week to two weeks, followed by the rehabilitation stage, which is usually one to two months. The fourth stage is health care, the fifth stage is health care, and the last stage is home. There are basically six stages." Zang Dawei said that Tianshan Rehabilitation Center focuses on the third stage, Carry out characteristic rehabilitation medicine.

In terms of specific technology, it covers all kinds of rehabilitation scenarios, from ultrasound guided botulinum toxin injection, balloon dilation, brain computer interface, to high-low frequency transcranial magnetic stimulation, transcranial DC stimulation, to lower limb exoskeleton robot, isokinetic muscle strength assessment and training, gait assessment and training, posture assessment and training technology, etc.

Qian Fang (alias) is the beneficiary of these rehabilitation techniques.

Qian Fang, over 70, was admitted to Xiaotangshan Hospital due to cerebral infarction and right hemiplegia. "Uncle Qian suffers from hypertension for a long time, his blood pressure is not well controlled, his white matter degeneration is very serious, and there is moderate brain atrophy, which has a big cognitive function problem. His left limb's motor function is also very poor, and he cannot stand or eat by himself. He always has a very serious expression, which is called 'mask face' in medicine." Xiao Juan said, "After he was admitted to the hospital, I arranged sports training, physiotherapy and cognitive function training for him as the doctor in charge."

Due to Qian Fang's poor cognitive function, he can't understand the instructions and can't cooperate with the active rehabilitation movement. Although the therapist always takes the trouble to demonstrate, explain and cooperate with him to carry out some traditional passive rehabilitation treatment, such as rehabilitation treadmill (passive mode), standing bed, etc., the treatment effect is not ideal.

After re evaluation, the center has reformulated the treatment plan for him and arranged repeated transcranial magnetic stimulation treatment, with the stimulation site in the bilateral frontal lobes, so as to improve Qian Fang's cognitive and motor functions.

"After three times of repeated transcranial magnetic stimulation treatment, a miracle occurred. Uncle Qian was able to eat with his right hand, and the meal time was significantly shorter than before. When he went to the treatment room for rehabilitation treatment, he was also able to actively cooperate with the treatment, and his subjective initiative was significantly improved. After about two months of treatment, he was able to walk with the help of others, and was discharged after gradually improving." Xiao Juan said.

Encourage medical staff to write parallel medical records

In recent years, China's rehabilitation treatment technology has made rapid development, but there are still some people who do not realize the importance of rehabilitation training, rely on drug therapy excessively, and some even refuse to accept rehabilitation training, especially for patients with serious dysfunction.

"Such patients are often in a state of anxiety and depression, do not accept the present results, and have little confidence in the future. So we should hurry up the treatment and recovery on the one hand, and encourage patients to improve their confidence on the other hand. If patients do not cooperate, many treatments may not work," said Zang Dawei, "As medical staff, we should more listen to the stories of these patients, learn to think in terms of empathy, and only by entering their inner world and understanding them can we deal with their problems symptomatically."

In this regard, Tianshan Rehabilitation Center starts from many aspects to create a comfortable and warm environment for patients. "We provide various auxiliary services, such as public wheelchairs, scanning and ordering meals, and volunteers provide haircuts, accompanying reading and other services." Kang Xiaoping said, "The hospital area is relatively large, and different diagnosis and treatment areas are far away. In order to reduce patients' walking, we provide ferry services."

In order to better empathize with patients, Tianshan Rehabilitation Center encourages medical personnel to write parallel medical records. "Let them listen to the patients' feelings and examine what problems they still have in the whole medical service, which can enhance their professional sense of honor and alleviate burnout." Kang Xiaoping said, "Many patients are discharged from hospital, and our rehabilitation therapists still keep in touch with them to guide the precautions of home health care."

"Serve patients and solve their physical and mental disorders. I think this is the significance of my existence as a rehabilitation doctor," said Xiao Juan.

The operation effect of Tianshan Rehabilitation Center provides experience and reference for follow-up cooperation with other departments. "Xiaotangshan Hospital will also continue to carry out close medical cooperation with Beijing Jishuitan Hospital, Beijing Children's Hospital, Beijing Hospital of Traditional Chinese Medicine and other municipal hospitals in orthopedic rehabilitation, children's rehabilitation, traditional Chinese medicine rehabilitation and other fields to further improve the comprehensive strength of Xiaotangshan rehabilitation medical platform," said Jiang Yue, "In this process, we will continue to strengthen humanistic care, enhance patients' sense of medical access, and push rehabilitation medicine to a new level of development."
