Pharyngitis of these people often becomes chronic and will not heal for a long time. What is the reason?


This reason is the "gastroesophageal reflux disease" which is easily ignored by some doctors and many ordinary people.
  On November 19, 2014, in Urumqi, I attended the seminar on "Gastroesophageal Reflux Disease" held by the People's Hospital of Xinjiang Autonomous Region I listened to academician Wang Zhonghao's introduction to gastroesophageal reflux disease.   Some scholars have reported that more than 75% of chronic pharyngitis is related to gastroesophageal reflux disease.
Lower esophageal sphincter (LES). When food enters the esophagus, it stimulates the mechanical receptors on the esophageal wall, which reflexively causes the relaxation of the lower sphincter, allowing food to enter the stomach. After the food mass enters the stomach, the lower esophageal sphincter contracts to restore its resting tension, which can prevent the stomach contents from flowing back into the esophagus.

When the pressure of the lower esophageal sphincter is insufficient (loose) or there is a hiatal hernia, the acid or food in the stomach will flow back into the esophagus, causing irritation symptoms (heartburn, acid regurgitation, belching, pain and discomfort in the xiphoid process and behind the sternum) and mucosal damage (esophagitis, esophageal ulcer, Barrett's esophagus, even esophageal stenosis and esophageal cancer) in the esophagus The stomach contents often flow back into the mouth to the pharynx, which can form fine particles or aerosols and be sprayed into the throat, inhaled into the trachea, bronchi and lungs, resulting in serious cough, expectoration, and even laryngeal spasm and dyspnea.

Heartburn, acid regurgitation, belching, pain and discomfort in xiphoid process and behind sternum are characteristic symptoms of gastroesophageal reflux disease. If a person has these symptoms within one week Gastroesophageal reflux disease can be diagnosed if it lasts for more than 3 weeks.
Fruits with high acidity, such as pineapple, willow, orange, bayberry, etc., will lead to the relaxation of the lower esophageal sphincter. When lying flat at night, stomach contents will flow back to the pharynx. The stimulation of gastric acid and bacterial infection will lead to acute pharyngitis. This is why eating too many oranges can lead to pharyngitis.

For pharyngitis of this kind of patients, the effect of simple anti-inflammatory treatment is not good. Gastroesophageal reflux should be treated first.

On November 19, 2014, at the seminar of "Gastroesophageal Reflux Disease" in Urumqi, some scholars proposed that all chronic pharyngitis could be diagnosed with "Gastroesophageal Reflux Disease". That is, ranitidine, famotidine or omeprazole are commonly used to inhibit gastric acid, and motiline, cisapride or mosapride are added to promote esophageal and gastric emptying and reduce reflux. On this basis, anti-inflammatory treatment often has good therapeutic effect.

  For this kind of patients, the general treatment should avoid mental stimulation, eat less, eat more meals, eat low fat, light diet, and avoid stimulating food; Overeating is not suitable, especially for dinner; Don't eat before bed; Avoid smoking, alcohol and coffee. Do not lie flat immediately after meals. When sleeping, raise the head of the bed to reduce the chance of acid reflux.

Of course, if there is pharyngitis caused by gastroesophageal reflux disease with hiatal hernia, doctor Sometimes surgical treatment of hiatal hernia is recommended to prevent reflux. This needs to be diagnosed in the hospital.
