1、   Briefly record the patient's gender, age, educational background, marital status and occupation;

2、   Reason for seeking medical advice: what problems were mentioned. Diagnosis of mental illness: who did it when? Does the patient try the drug?

3、   Briefly record your first impression of the patient;

4、   What problems and pains did the patient mention during the first treatment? How the patient described him / Her problems and pain?

5、   Early life background: important others and important experiences. Just give a clear summary and don't need to describe too much detail.

Section one To five Item, for case report ten About minutes.

6、   Treatment settings: protocol, treatment times, frequency and cost.

7、   Treatment process: briefly describe the treatment process so far.

8、   Briefly describe your opinion on the patient's problem: how the problem came into being and how it is now. What problem do you think the patient is struggling with.

Section six To eight Item, time five About minutes.

9、   Dialogue: regarded as a detailed report, and attached with one or two conversations during treatment. This is very important. It can provide detailed and valuable discussion materials for treatment work (methods and techniques) in supervision.
