The accurate name of serum creatinine (Cr) is blood creatinine. Blood creatinine is generally considered as endogenous creatinine, which is the product of human muscle metabolism. In muscle, creatine is slowly formed into creatinine mainly through irreversible non enzymatic dehydration reaction, and then released into the blood and excreted with urine. Therefore, blood creatinine is closely related to the total amount of muscle in the body and is not easily affected by diet. Creatinine is a small molecular substance that can be filtered through the glomerulus and rarely absorbed in the renal tubules. Almost all creatinine produced in the body every day is discharged with urine and is generally not affected by urine volume. Clinical detection of serum creatinine is one of the main methods to understand renal function and an important indicator of renal function. The increase of serum creatinine means the damage of renal function.

1. Increased serum creatinine
(1) Acute and chronic glomerulonephritis, nephrosclerosis, polycystic kidney, rejection of kidney transplantation, etc. For patients with chronic nephritis, the higher the serum creatinine content, the worse the prognosis. Creatinine concentration increased in uremia and severe congestive heart failure.
(2) Dehydration, blood loss, shock, heart failure, intense physical activity, acromegaly and gigantism, and increased serum creatinine.
2. Decrease of serum creatinine
Muscle atrophy, severe liver disease, leukemia and renal insufficiency.
3. Serum creatinine concentration is used for decomposition of chronic renal insufficiency
Phase I (compensatory phase of renal insufficiency): GFR 50 ~ 80mL/min; Scr133~177μmol/L。
The second stage (decompensation stage of renal insufficiency): GFR50 ~ 20mL/min; scr178~442μmol/L。
The third stage (renal failure stage): GFR20 ~ 10mL/min; scr443~707μmol/L。
The fourth stage (uremic stage): GFRmin; scr>707μmol/L。
