Spirochete is a kind of prokaryotic microorganism which is slender, soft, curved and active. In biological position, it is between bacteria and protozoa. Spirochetes are widely distributed in nature, commonly found in water, soil and decaying organic matter, and also in human oral cavity or animal body.
There are mainly three genera that are pathogenic to humans: ① Leptospira: Leptospira is the main pathogenic to humans; ② Treponema: Treponema pallidum is the main human pathogen; ③ Leptospira: It is mainly retrogressive fever spirochetes that cause disease to people. Leptospira and Treponema pallidum have a greater impact on clinical practice.

    Form and structure: The spirochete is thin and consists of spiral columnar protoplasts, internal flagella and outer membrane. The diameter is generally 0.1~0.3 μ m, with different lengths and helicoids, but the leptospira is C or S shaped. Spirochaeta Gram staining is negative, but not easy to stain, so Fontana silver plating staining method is commonly used.

The pathogenesis of spirochetes is still unknown. Different spirochetes cause diseases through different pathogenic substances and mechanisms. Leptospira may be related to its hemolysin, cytotoxic factors and endotoxin like substances that dissolve red blood cells. The pathogenesis of Treponema pallidum is related to the adhesion of membrane like substances on its surface and the action of mucopolysaccharidase secreted by it.

The immunity of spirochetes is mainly humoral immunity. Long term humoral immunity to leptospirosis can be obtained after the recovery of leptospirosis. The immunity of Treponema pallidum is infectious immunity. The specific antibody produced by the infected body can kill or dissolve Treponema pallidum.

Microbiological examination of spirochetes mainly includes pathogen examination and serological test.
