Pulmonary arteriography

Pulmonary angiography includes selective and non selective. Non selective pulmonary angiography appeared in the early 1930s, when contrast agents were injected into peripheral veins to make pulmonary vascular imaging. Selective pulmonary angiography was adopted in 1963. After nearly 40 years of development, this technology has gradually become a safe and effective diagnostic technology, which has helped to diagnose a variety of congenital or acquired pulmonary vascular diseases. Pulmonary angiography is also highly valued because it is not only a diagnostic process, but also an important means of interventional treatment, such as mechanical thrombolysis of pulmonary artery thrombosis, pulmonary angioplasty, and percutaneous catheter embolization of pulmonary arteriovenous fistula. However, because pulmonary angiography requires certain technology and equipment, which is an invasive examination with certain complications and mortality, its application is limited. At present, the main purpose of selective pulmonary angiography is to be used for the differential diagnosis of emergency interventional surgery and difficult cases.

1. Explain the purpose of imaging and possible complications and accidents to patients and their families, and sign an imaging agreement.
2. Ask about the medical history and necessary imaging examinations, such as chest film, CT, fiberoptic bronchoscopy, etc.
3. Check the function of heart, liver and kidney, as well as blood routine test and coagulation time.
4. Iodine and anesthetics shall be treated as required by the pharmacopoeia.
5. Do not eat or drink 4 hours before operation, and intramuscular injection of diazepam (diazepam) 10mg 0.5 hour before operation.
6. Routine skin preparation at puncture site.
7. If necessary, measure arterial partial oxygen pressure or blood oxygen saturation.
8. Establish venous channels to facilitate intraoperative medication and rescue.

Pulmonary arteriography refers to an examination in which pig tail catheter is used to pass through the inferior vena cava, super selectively intubate to the main trunk of the left and right pulmonary arteries, inject contrast agents into the pulmonary arteries to make the pulmonary arteries develop, so as to understand the location of pulmonary vascular lesions, determine the nature of the lesions, and select appropriate embolic materials for treatment according to the condition. It is mainly used to diagnose and treat the following diseases:
1. Pulmonary aneurysm and pulmonary arteriovenous fistula, understand the size and opening position of aneurysm and fistula, and use spring coil and liquid glue to block the aneurysm or fistula.
2. Congenital pulmonary arteriovenous malformations. Angiography shows the size and location of vascular malformations, and spring coils and liquid glue are used for treatment.
3. Acute pulmonary embolism: understand the degree and location of pulmonary embolism, use pig tail catheter for mechanical smashing, injection of thrombolytic drugs and balloon dilation of the stenosis artery to achieve the therapeutic purpose.
