2. Acceptance procedure
The applicant shall fill in the Application Form for Government Information Disclosure of Hubei Provincial Environmental Protection Department (hereinafter referred to as the Application Form) when applying to Hubei Provincial Environmental Protection Department for government environmental information disclosure. The Application Form shall be completed completely and the contents shall be true and effective. The Application Form can be downloaded from the website of Hubei Provincial Environmental Protection Department or obtained from the acceptance agency of Hubei Provincial Environmental Protection Department. The copy of the Application Form is valid.
When an individual applies, please provide a copy of his/her ID card. When a legal person or other organization applies, please also provide a copy of the organization code certificate or the business license. The applicant shall be responsible for the authenticity of the application materials.

3. Application method

(2) Apply orally in person. 4. Content of application

5. Application processing
(1) Registration
For applications that have been completely filled in the Application Form and the applicant has provided a valid identity certificate, the receiving agency shall register immediately.
(2) Review
After receiving the application, this organ will formally review whether the elements of the application are complete. Applications with incomplete elements will be returned and the applicant will be required to correct the information.
(3) Reply
The acceptance institution of Hubei Provincial Environmental Protection Department shall reply on the spot if it can reply on the spot after receiving the government information disclosure application. If the reply cannot be made on the spot, it shall be made within 15 working days from the date of receiving the application (if it is really necessary to extend the reply time limit, the extension of the reply time shall not exceed 15 working days with the consent of the person in charge of the government information disclosure work organization of the Provincial Environmental Protection Department, and the applicant shall be informed).
① If it is within the scope of disclosure, inform the applicant of the information content or the way and means of obtaining information;
② If the government information applied for disclosure contains information that should not be disclosed, but can be handled differently, Hubei Environmental Protection Department shall provide the applicant with information that can be disclosed;
③ If it falls within the scope of exemption from disclosure, the applicant shall be informed of the reasons for not disclosing it;
④ If the information does not exist or does not belong to the work scope of Hubei Provincial Environmental Protection Department, inform the applicant that the information does not exist or other ways and means of searching;
⑤ If the content of the information applied for disclosure is not clear, the applicant shall be informed to correct the content of the application and reapply for information disclosure.
6. Fee items and standards for providing information according to the application
If the government information is provided according to the application of the applicant through paper, disk, CD and other media, the retrieval fee can be charged according to the number of pieces of government information provided; If materials need to be copied, printed, recorded and copied, and paper, disk, CD and other items are used, corresponding costs will be charged; Materials delivered by mail or delivery will be charged according to national standards. If environmental information disclosure is provided to the applicant in other forms and charges are required, it shall be implemented after approval.
The cost of copying, mailing and delivering the actively disclosed environmental information at the request of the applicant may be subject to the above charging standards. If the applicant applies for fee reduction or exemption, please provide relevant proof at that time.
3、 Complaint supervision
Citizens, legal persons or other organizations may complain to the supervision department if they believe that the Environmental Protection Department of Hubei Province has failed to perform its obligation to disclose government information in accordance with the law, and the receiving institution has failed to timely handle the disclosure of government information in accordance with the application.
