1、 Identification object and acceptance scope
（1） Identified object
Those who have not reached the national statutory retirement age, meet the application conditions stipulated by the state and our city, have participated in the national primary and secondary school teacher qualification examination and obtained the Certificate of Qualification for Primary and Secondary School Teachers within the validity period, or have obtained the Certificate of Professional Competence for Teachers of Normal Students and within the validity period (educational postgraduates and normal students included in the reform scope of examination exemption recognition) 。 If the above conditions are met, the personnel applying for accreditation in our district shall also meet one of the following conditions:
1. Chinese citizens who have registered in the district or hold valid residence permits in the district;
2. Hong Kong, Macao and Taiwan residents who study, work and live in this area with a Hong Kong, Macao and Taiwan resident residence permit, or who hold a Hong Kong, Macao and Taiwan resident travel permit to the mainland, a five-year Taiwan resident travel permit to the mainland, and who take the primary and secondary school teacher qualification examination in Sanming and live in this area.
（2） Acceptance scope
According to the relevant national regulations, the education bureau of our district is responsible for the accreditation of kindergarten, primary school and junior high school teachers through online registration.
The education bureau of our district is responsible for the on-site confirmation and collection of materials for applying for the qualification of senior middle school, secondary vocational school and secondary vocational school internship instructor in our district, and the education bureau of Sanming City is responsible for the identification.
2、 Confirmation time
（1） Online registration: 08:00, October 7, 2023 to 18:00, October 16, 2023. If the opening time of the system is adjusted, please refer to the notice of China Teacher Qualification Network.
（2） Physical examination: October 7 to October 18, 2023 (normal working hours).
（3） On site confirmation: October 23 to November 3, 2023 (normal working hours).
In order to improve work efficiency, the applicant is requested to go to the site for confirmation according to the following schedule. If the applicant for accreditation of teachers in primary and secondary schools and kindergartens misses the confirmation time arranged in the following corresponding school sections, he/she can also make on-site confirmation before the deadline for on-site confirmation in our district (November 3).
Kindergarten: October 23 to October 25, 2023;
Primary school: October 25 to October 27, 2023;
Junior high school: October 30 to November 1, 2023;
Internship guidance for senior middle schools, secondary vocational schools and secondary vocational schools: November 1 to November 3, 2023.
（4） It is verified that: before November 30, 2023 (normal working hours).
（5） Obtaining the certificate: the confirmation result and the specific time for obtaining the certificate will be published on the official website of the District Education Bureau (website: http://www.fjsx.gov.cn/wzq/bmwz/jyj/ ）Notice, please pay attention to it in time.
3、 Recognition conditions
（1） Abide by the Constitution and laws, love education, fulfill the obligations stipulated in the Teachers Law of the People's Republic of China, and abide by teachers' professional ethics.
（2） They have good physical and psychological quality, can meet the needs of education and teaching work, and comply with the provisions of the Notice of Fujian Provincial Department of Education and Fujian Provincial Health and Family Planning Commission on Printing and Distributing the Physical Examination Standards and Measures for Teachers' Qualification Applicants in Fujian Province (revised in 2018) (Min Jiao Shi [2018] No. 20), In our district, the designated Grade II Grade A medical institution (North District of the District General Hospital) passed the physical examination.
（3） Have the corresponding educational requirements specified in the Teachers Law of the People's Republic of China.
1. To apply for recognition of kindergarten teacher qualification, one should have graduated from a kindergarten normal school or above;
2. To apply for the qualification of a primary school teacher, one should have graduated from a secondary normal school or above;
3. To apply for recognition of the qualification of a junior high school teacher, one should have graduated from a higher normal college or another college or above;
4. Applicants who apply for recognition of the qualifications of teachers in senior middle schools and teachers in secondary vocational schools and technical schools should have a bachelor's degree or above from a normal university or another university;
5. To apply for the certification of the internship instructor qualification of a secondary vocational school, one should have graduated from a college or above, and should also have a professional and technical position of assistant engineer or above, or a technical level of intermediate workers or above.
The valid academic qualifications recognized by teachers' qualifications refer to the national education series of academic qualifications recognized by the education administration department of the State Council, which can be found on the "China Higher Education Student Information Network"（ http://www.chsi.com.cn ）Query or pass the certification of "National University Student Information Consulting and Employment Guidance Center". The academic degree obtained abroad can be certified by the "Overseas Study Service Center of the Ministry of Education", and can be regarded as a qualified academic degree to recognize the corresponding teacher qualifications.
（4） Reaching the second grade B and above of the Standard for the Level of Putonghua Proficiency Test issued by the National Language and Character Working Committee; Among them, Chinese teachers and teachers of teaching Chinese as a foreign language should reach Grade II Grade A or above.
4、 Identification procedure
（1） Online declaration
Online registration: online registration is applied for teacher qualification recognition. Qualified applicants log on to the China Teacher Qualification Network (www.jszg. edu. cn) within the specified online registration time, and click "online processing" to register and recognize their accounts through the "teacher qualification recognition" service entrance under the "online service" column. The same applicant can only apply for one teacher qualification at a time, and cannot apply for a second teacher qualification certificate within one natural year after successful application.
If a teacher's qualification is revoked, he shall not reapply for recognition of teacher's qualification within five years from the date of revocation; Those who have been deprived of their political rights or who have been subjected to a criminal punishment of not less than fixed-term imprisonment for intentional crimes cannot apply for recognition of teachers' qualifications.
matters needing attention:
1. Register online newspaper. When reporting online, please register your identity according to the system prompts, fill in and submit the application information on the web page. Applicants should carefully read the online declaration operation tips of China Teacher Qualification Network, and fill in the personal declaration information truthfully and accurately according to the online declaration process and tips. If the teacher's qualification cannot be recognized due to the untrue and inaccurate information, the responsibility shall be borne by the applicant.
2. Select the type of location where the applicant is located. The applicant must correctly select the type of application location according to his/her actual situation: "domicile" (refers to the residence within the jurisdiction of Shaxian District) or "residence" (the residence is not within the jurisdiction of Shaxian District, but in Shaxian District, he/she must hold a valid residence permit), and accurately fill in the location information according to the residence or residence permit. Applicants who fail to register according to the operation requirements will not be able to pass the on-site confirmation review.
3. Select the on-site confirmation point. Apply for teachers' qualifications in our district, apply for teachers' qualifications in kindergartens, primary schools and junior high schools, and select "Shaxian District Education Bureau" as the accreditation institution and on-site confirmation point; To apply for the qualification of internship instructor in senior middle schools, secondary vocational schools and secondary vocational schools, the accreditation institution shall select "Sanming Education Bureau", and the on-site confirmation point shall select "Shaxian District Education Bureau". After online reporting, the applicant will not be able to modify the on-site confirmation point, and the identification will not be possible if the on-site confirmation point is incorrectly selected.
4. Personal commitment. The teacher ethics commitment system is fully implemented in the application for teacher qualification. The applicant should scan the code and sign according to the system prompts.
5. Select the examination form. The applicant's wrong selection will not be recognized normally. Those who hold the Primary and Secondary School Qualification Examination Certificate and are within the validity period shall choose the "National Unified Examination"; Educational postgraduates and normal students who hold the Professional Competence Certificate for Teachers of Normal Students and are within the validity period shall choose "reform personnel recognized without examination".
6. Check the information. After the online declaration is completed, you should log in to the online reporting system again to check the current declaration status, information and on-site confirmation precautions of individuals. Please be sure to pay attention to the online declaration status of individuals and the message information given by the accreditation agency. If you encounter problems, please refer to the "Frequently Asked Questions" column description and "Use Help" on the home page of China Teacher Qualification Website.
（2） Physical examination
After successful online application, make an appointment for physical examination at the Physical Examination Center of Shaxian District General Hospital (North District) (Tel: 0598-5841995) within the specified physical examination time.
The medical examination fee shall be borne by the applicant and collected according to the routine medical examination standard of the hospital. If the applicant does not go to the designated medical examination hospital for medical examination within the specified time, the conclusion shall be deemed invalid. The physical examination results (physical examination forms) shall be uniformly returned to the hospital by Shaxian District Education Bureau, and any other examination materials obtained by the applicant shall not be used as the basis for teacher qualification verification. The current physical examination report is only valid for the current identification.
1. Physical examination process:
The applicant takes a photo (one recent bareheaded two inch white background color photo on the same page as the online newspaper) and ID card to the physical examination center of Shaxian District General Hospital (North District) to get the uniformly printed Physical Examination Form for Teachers' Qualification Applicants in Fujian Province (the physical examination form has a number and is invalid if printed) within the appointment time, According to the requirements of the cover of the physical examination form, fill in the personal information truthfully with a black pen (including kindergarten, primary school, junior high school, senior high school, secondary vocational school and secondary vocational school internship guidance for the "type of application qualification"), paste the photos, fill in the form, issue a bill according to the relevant charging standards, and check the real name of each department according to the procedures specified by the hospital before conducting the physical examination.
2. Precautions:
(1) To apply for kindergarten teacher qualification, additional tests are required. When receiving the form, each applicant should inform the staff of the physical examination center of the type of teacher qualification applied for and receive the corresponding physical examination form for billing, so as to avoid wrong charges. After the examination, the physical examination form must be returned to the designated place of the physical examination center. If all items cannot be completed on the same day, the physical examination form must be delivered to the physical examination center for safekeeping. Please record the number by yourself, so that the confirmation staff can be informed to inquire during on-site confirmation.
(2) You must have an empty stomach before the physical examination, and do not eat after 20:00 of the previous day. In order to improve the conclusion of physical examination, the physical examination institution may require individual applicants to conduct further examination on additional physical examination items with missing items or inaccurate and unclear conclusions.
(3) Pregnant women should inform the medical staff in advance not to have chest radiography. If the physical examination items are not completed due to personal reasons (including the inability to complete chest radiography due to pregnancy, etc.), according to the provisions of the document of Fujian Provincial Department of Education, it will not be able to issue a conclusion of physical examination and participate in the current teacher qualification certification.
(4) Those who have a medical history of major diseases (refer to those who have been hospitalized for treatment or major surgery or need to take medicine for a long time), please bring relevant medical records and examination and other supporting materials, and inform the doctor of the medical history of major diseases and trauma surgery history. If the medical history is concealed and affects the physical examination results, or the medical examination is not performed according to the doctor's instructions, the consequences will be borne by yourself.
(5) Some examination items may require on-site appointment in advance. In order to complete all the examination items in time before the on-site confirmation stage and obtain complete conclusions, the applicant is requested to arrange a reasonable time to participate in the physical examination as soon as possible. Do not wait until the deadline for the physical examination is near or the same day to participate in the physical examination.
(6) In the process of physical examination, the applicant should strictly abide by the rules and regulations of the hospital, follow the guidance of the doctor, cooperate with the work of nurses and staff, and strictly prohibit fraud and impersonation. If any violation is found, it will not be recognized, and administrative punishment will be imposed according to "fraud to obtain the qualification of teachers".
（3） On site confirmation
After the online application is successful, the applicant must submit the materials related to the application for teacher qualification recognition to the on-site confirmation point within the on-site confirmation time, and confirm the online information, otherwise the application is invalid. If the applicant fails to submit the materials to the site for confirmation within the specified time, it will be deemed to have automatically abandoned the application. If I cannot go to the site for confirmation due to special reasons, I can entrust others in writing to go to the site for confirmation (see "V. Other Precautions" for the entrusted materials).
1. Location of on-site confirmation: Personnel Unit of Education Bureau of Shaxian District (Tel: 0598-5855809).
2. Materials and requirements to be submitted by the applicant during on-site confirmation:
(1) Second generation ID card within the validity period. If the applicant entrusts others to handle the case, the agent must provide the original ID card of the agent, submit the power of attorney and one copy of the ID card of the applicant and the agent.
(2) Household registration certificate or identity certification materials.
① Applicant of local household registration: provide the original of the applicant's Household Register. In case of collective household registration in this district, one copy of the collective household registration book, including the home page of the household registration book and the individual page of the applicant, shall be provided. The copy of the collective household personnel shall also be stamped with the official seal of the unit where the collective household is located;
② Non local household registration applicant: provide the original and one copy of Shaxian District's valid residence permit;
③ Hong Kong, Macao and Taiwan applicants: provide the original Hong Kong, Macao and Taiwan resident certificate issued by the district. Hong Kong, Macao and Taiwan residents who have passed the primary and secondary school teacher qualification examination in Sanming City shall provide: Hong Kong and Macao residents shall provide the original mainland travel permit, and Taiwan residents shall provide the original mainland travel permit for Taiwan residents with a five-year validity.
(3) Photos. Submit one recently bareheaded front 1-inch color white background photo and one electronic photo of the applicant (required to be consistent with the photo uploaded in this accreditation application). Please use a ballpoint pen on the back of the paper photo to indicate the name of the confirmation point, the name and the identified subject. The paper photo is used to produce the certificate, and must have a reasonable proportion of portraits, the size meets the standard, and the image is clear; Electronic photo specification: less than 190k, the picture is in jpg format, the resolution width is more than 290 pixels and less than 300 pixels, and the height is more than 408 pixels and less than 418 pixels. Please name the electronic photo with "qualification type, discipline+name+ID card number" (such as: junior high school geography+Zhang San+350427 ***********************************************) and send it to the email in the form of attachment before on-site confirmation sxjszgrd@163.com 。
(4) Diploma. The certificate information must be verified online. If the academic information is successfully verified online through the application system, there is no need to submit paper materials; If you are unable to successfully retrieve academic information to complete online verification, please upload photos according to the system prompts, and provide 1 original and copy of the corresponding certificate at the time of on-site confirmation, as well as China Higher Education Student Information Network (Xuexin) (website: http://www.chsi.com.cn ）The "Electronic Registration and Filing Form of Academic Credentials of the Ministry of Education" or "China Higher Education Academic Credentials Certification Report" issued by the Ministry of Education.
For education in Hong Kong, Macao and Taiwan, the original and one copy of the Certificate of Education in Hong Kong, Macao and Taiwan issued by the Overseas Study Service Center of the Ministry of Education shall be submitted at the same time. For foreign education, the original of the Certificate of Education in Foreign Countries issued by the Overseas Study Service Center of the Ministry of Education shall also be submitted at the same time. Otherwise, it will be deemed as unqualified educational background and will not be accepted. It is suggested that the applicant submit the Electronic Registration and Filing Form of Educational Certificate of the Ministry of Education, and extend the use verification period appropriately on the Xuexin website to ensure that the verification is successful, so as not to affect the identification.
(5) Certificate of Qualification for Primary and Secondary School Teachers or Certificate of Professional Competence for Teachers of Normal College Students. The certificate information must be verified online, and those passing the verification may not be submitted; Those who can not complete the verification shall go to the primary and secondary school teacher qualification examination website（ https://ntce.neea.edu.cn/ ）Print the paper certificate and submit it to the site for confirmation; If the Certificate of Professional Competence for Teachers of Normal Students cannot be verified online, the applicant should confirm with the university.
(6) Mandarin level test certificate. The certificate information must be verified online, and those passing the verification may not be submitted; If the online verification is not successful, please upload the photos according to the system prompt requirements, and provide the original and copy of the corresponding certificate at the time of on-site confirmation.
(7) The original of Physical Examination Form of Fujian Teachers' Qualification Applicants. The district education bureau will collect the medical examination form from the medical examination hospital (the applicant can record the number of the medical examination form and inform the staff to verify the conclusion).
(8) Other evidentiary materials required for teacher qualification recognition.
① In case of applying for the qualification of an internship instructor in a secondary vocational school, the original and one copy of the certificate of professional and technical title of assistant engineer or above or the certificate of technical grade of intermediate workers or above shall also be provided.
② Hong Kong, Macao and Taiwan residents shall submit a certificate of no criminal record issued by the relevant departments of Hong Kong Special Administrative Region, Macao Special Administrative Region and Taiwan. If necessary, the applicant can apply for the relevant letter for the certificate of no criminal record at the on-site confirmation point.
Note: The originals of materials 4, 5 and 6 that have passed the system comparison and verification are not required for online registration. If the materials submitted by the applicant are incomplete or not in conformity with the requirements, they shall be supplemented before the end of the on-site confirmation time, otherwise the identification will not be carried out normally. The above originals (excluding certification materials and medical examination forms) shall be returned to the applicant after review. All printed copies and photocopies shall be in A4 paper. The applicant shall arrange them in order and submit them together with a paper clip in the upper left corner for confirmation.
（4） Verification and certificate issuance
After the on-site confirmation, our bureau will submit the relevant materials of applicants for senior high school, secondary vocational school and secondary vocational internship instructor qualifications to the Sanming Municipal Education Bureau. The accreditation institution will complete the review and accreditation before November 30, 2023, and issue teacher qualification certificates and Teacher Qualification Accreditation Application Form to the applicants who meet the accreditation conditions. After the applicant finds the certificate information on the China Teacher Qualification Website, please go to the on-site confirmation point to get the teacher qualification certificate and the Application Form for Teacher Qualification Accreditation within the time specified by the Shaxian District Education Bureau in the official website accreditation result notice. Please pay attention to the notice on the website of Shaxian District Education Bureau for the specific collection time.
Note: The applicant should timely put the "Application Form for Teacher Qualification Accreditation" he received into his personal file.
5、 Other precautions
（1） The applicant shall timely consult the notice notice issued on the website of the accreditation institution, and make online declaration, participate in physical examination and on-site confirmation according to the time, place and requirements specified by the accreditation institution. If the application is not completed within the specified time due to missing the application time, selecting the wrong identification institution or on-site confirmation point, incorrect application information or incomplete submission of materials, the identification institution will be unable to accept the application, and the responsibility will be borne by the applicant himself.
（2） Applicants shall truthfully submit relevant materials, deliberately practice fraud, and those who cheat teachers will be punished according to relevant national regulations.
（3） In accordance with the requirements of the Opinions of the Ministry of Education and the Ministry of Public Security of the Supreme People's Procuratorate on Establishing a System for Teachers and Staff Access to Query Information on Sexual Offences, the Ministry of Education makes overall arrangements, and the public security department assists the education department to carry out information inquiries, and implements the system for teachers and staff access to query information on sexual offences. Those who fail to pass the admission will not be recognized as teachers.
