When applying for arbitration, the parties shall go through the filing formalities in the receiving room and submit the following materials:
1. Original arbitration application.
2. The identity cards of the applicant and the respondent.
3. Power of attorney of the entrusted agent.
4. Arbitration agreement.
5. Other evidentiary materials.
[Legal Basis]
In Article 23 of the Arbitration Law, the arbitration application shall contain the following items:
(1) The name, sex, age, occupation, work unit and address of the party concerned, the name and address of the legal person or other organization, and the name and position of the legal representative or principal responsible person;
(2) The arbitration claim and the facts and reasons on which it is based;
(3) Evidence and its source, name and address of witness.
