The sick leave certificate can be supplemented and the sick leave procedures can be improved. If employees need to take leave due to injury or illness, they shall present the disease diagnosis certificate issued by the enterprise medical institution or designated hospital, which shall be reviewed and approved by the enterprise.
 The sick leave form is issued by the hospital. If it is lost, it needs to be issued again through negotiation with the hospital. However, it can only be issued according to the date of the original rest certificate, without changing the original time node.

 Article 23 of the Law of the People's Republic of China on Medical Practitioners stipulates that doctors who take medical, preventive and health care measures and sign relevant medical certificates must make diagnosis and investigation in person, fill in medical documents in a timely manner according to regulations, and may not conceal, forge or destroy medical documents and relevant materials.
 Doctors shall not issue medical certificates irrelevant to their scope of practice or inconsistent with the type of practice.
