The procedure of reissue is that parents bring their ID cards, household register and marriage certificate to the hospital when the child is giving birth and issue the "medical certificate of childbirth"; Then apply to fill in the "Application Form for Re issuance of Medical Certificate" at the Maternal and Child Health Care Center; Finally, apply to the local health administrative department for a replacement.
Legal basis
Article 23 of the Law of the People's Republic of China on Maternal and Infant Health Care, medical and health institutions and personnel engaged in family midwifery shall, in accordance with the provisions of the administrative department of health under the State Council, issue a unified medical certificate for newborn birth; If there are maternal and infant deaths and birth defects of newborns, they should report to the administrative department of public health.
