1、 Materials required for applying for injury identification include:
 1. ID card or account book;
 2. Power of attorney for disability appraisal;
 3. Admission records;
 4. Discharge records;
 5. Discharge summary;
 6. Medical record book;
 7. Disease diagnosis certificate;
 8. X-ray, CT and diagnostic report at the initial stage of injury and after the end of treatment;
 9. Other required materials.
 2、 The identification of injury and disability refers to the identification of the degree of disability after injury. The scope of injury and disability appraisal includes traffic accident disability, work-related injury disability, accidental injury disability, and fighting disability. Generally, the judicial departments (such as the traffic police, police stations, and courts) make corresponding disability identification according to the relevant admission records provided by the hospital or entrust the disability identification agency.

 According to Article 18 of the Provisions on Public Security Organs Handling Injury Cases, the public security organs shall issue a letter of authorization for injury identification within 24 hours after accepting the injury cases, and inform the victims to go to the designated identification institution for injury identification.
