To change a driver's license, you need: the driver's identity certificate, the original motor vehicle driver's license, the completed Application Form for Motor Vehicle Driver's License, and three recent bareheaded, half length, one inch photos of the driver. According to Article 60 of the Provisions on the Application and Use of Motor Vehicle Driving Licenses, the motor vehicle driver shall apply for renewal of the license at the place where the motor vehicle driving license is issued or the vehicle management office outside the place where the license is issued within 30 days under any of the following circumstances: (1) The information of the motor vehicle driver recorded in the motor vehicle driving license has changed within the jurisdiction of the vehicle management office; (2) The motor vehicle driver's license is damaged and unrecognizable. When applying, the applicant shall fill in the application form and submit the identity certificate and motor vehicle driver's license of the motor vehicle driver.
