When the objector raises an objection to the execution or the reconsideration applicant applies for reconsideration, he shall submit an application to the people's court. The application shall specify the specific objection or request for reconsideration, facts, reasons, etc., and attach the following materials:
（1） The identity certificate of the objector or the applicant for reconsideration.
（2） Relevant evidence materials.
（3） Delivery address and contact information.
