In case of a traffic accident claim dispute, the following materials shall be prepared for filing a claim for traffic accident compensation:
 （1） Evidence proving the subject qualification of the plaintiff and defendant
 1. If the plaintiff is a natural person, he/she shall submit his/her identity certificate, such as ID card, household register, etc.
 2. If the plaintiff is a legal person or other organization, it shall submit the registration materials of the subject, such as the copy of the industrial and commercial business license or the industrial and commercial registration list issued by the industrial and commercial registration authority, and the registration certificate of the association legal person.
 3. If the plaintiff has changed its name after the occurrence of the legal facts in dispute, it shall submit the change registration materials.
 4. If the party concerned is the family member of the deceased in the road traffic accident damage compensation dispute, the proof of the first heir of the deceased and the proof of the basic information of the heir shall be submitted.
 5. Submit the certificates of the owner, actual controller and driver of the vehicle involved in the accident and the certificates of relevant relationships.
 6. Provide certificates of the owner, actual controller and driver of the damaged vehicle as well as the mutual relationship.
 （2） Evidence proving that traffic accidents have litigation and agency qualifications
 1. If the plaintiff is a legal person or other organization, the identity certificate of the legal representative or the certificate of the person in charge (with official seal) shall be submitted.
 2. If the plaintiff entrusts others to act on his behalf in litigation, he shall submit a power of attorney signed or sealed by the client. The power of attorney shall specify the entrusted matters, authority, duration and contact number, and submit a copy of the agent's identity certificate. If the agent is a practicing lawyer, he shall submit a copy of his practicing license.
 3. If the plaintiff is a Hong Kong resident or a party related to Taiwan, Macao and other regions or countries entrusts a domestic practicing lawyer to act on behalf of the plaintiff in litigation, the notarization entrustment procedures shall be handled by a Hong Kong lawyer entrusted by the Ministry of Justice in accordance with the provisions of the Ministry of Justice.
 （3） Evidence proving the traffic accident and other evidence proving the claim of both parties
 1. Traffic accident damage compensation lawsuit should be submitted to the public security and traffic management department for handling traffic accidents
 Certificate of responsibility determination, mediation statement for traffic accident compensation, or mediation ending accident
 2. Forensic identification materials, traffic accident disability assessment or disability re assessment.
 3. In case of personal injury, the disease certificate, discharge certificate and transfer certificate issued by the hospital shall be submitted. Invoice of medical expenses, certificate of salary for the injured, certificate of salary of nursing staff, certificate of price of disabled equipment, certificate of basic information of the provider, invoice of transportation and accommodation expenses, etc.
 4. In case of financial damage, submit financial loss assessment report and maintenance invoice.
 5. Provide the calculation method and list of the specific claim amount in the claim.
