 1. The Application Form for Registration of Self employed Businesses signed by the business operator;
 2. Copy of the operator's ID card (front and back copies);
 3. Certificate of use of business premises:
 Where an individual industrial and commercial household uses its own premises as its business premises, it shall submit a copy of the certificate of ownership of its own premises; In case of renting the premises of others, a copy of the lease agreement and the property right certificate of the premises shall be submitted; If the property right certificate of the business site cannot be submitted, relevant certificates issued by the market sponsor, various development zone management committees and village neighborhood committees approved by the government to engage in business activities in the business site can be submitted; If the military property is used as the residence, a copy of the Military Real Estate Lease License shall be submitted.
 If the residential house is changed into a commercial house, if it is an urban house, it shall also submit the Registration Schedule - Residence (Business Site) Registration Form and the certification document issued by the local residents' committee (or owners' committee) that the interested owners agree to change the residential house into a commercial house; For non urban houses, relevant certificates required by the local government shall be submitted;
 4. If the business scope applied for registration includes items that must be reported for approval before registration as required by laws, administrative regulations and decisions of the State Council, a copy of the relevant license or approval document shall be submitted;
 5. Notice on Pre approval of Individual Business Name (Application for Pre approval of Individual Business Name is not required if there is no shop name or business scope does not involve pre licensing items);
 6. If an agent is entrusted to handle the transaction, the Certificate of Entrusted Agent signed by the operator and the photocopy of the ID card of the entrusted agent shall also be submitted.
