In real life, 4050 applications are generally applied offline. The application process is as follows: 1. After paying the pension and medical insurance of this year, the applicant takes his/her ID card, custody contract and the original payment note three days later, makes a preliminary examination at the social insurance subsidy acceptance window of the flexible employment personnel in the Municipal Archives Trusteeship Center, and receives the Application Form and flexible employment certificate. 2. The sponsor shall issue a flexible employment certificate at the community labor security workstation where he/she is registered and sealed. 3. The applicant takes the Application Form, Flexible Employment Certificate, his/her ID card, household registration book and other relevant materials (in triplicate) to the street (township) labor security office to apply for review, and signs and seals after the review is correct (the labor security office keeps one copy). 4. The applicant takes the Application Form, Flexible Employment Certificate and other relevant materials to the acceptance window of the Municipal Archives Trusteeship Center to go through the declaration procedures. 5. The Municipal Archives Trusteeship Center shall check the applicant's files, check the applicant's specific information and submit it to the labor and social security department.
