1. Household register or household registration certificate;
2. Original ID card;
3. Two recent two inch half length bareheaded color photos;
4. Divorced people should bring divorce certificates or court mediation (judgment) papers, widowed people should bring death certificates issued by public security organs;
5. In case of entrustment, the power of attorney signed by the party concerned and the ID card of the entrusted person shall also be submitted. Be sure to note that the form of the power of attorney must not be wrong, and the elements must be complete. Before going to the local police station where the household registration is located to issue a certificate, it is better to confirm it by phone, because the local police stations may require different documents in addition to important documents.
