 Traffic accident materials shall be objective, true and able to comprehensively reflect the accident situation. The format can be referred to as follows:
 1. The name, gender, age, work unit or home address, ID card number, contact number, postal code and other basic personal information of the party concerned.
 2. The time and place of the accident, the important environment, weather and road conditions around the site.
 3. The mode of transportation of the party and the other party, the direction of the vehicle, and the destination to be reached.
 4. Road conditions and safety measures taken in case of danger.
 5. The type of driving permit, handling time, safety and technical conditions of the vehicle, vehicle type and license plate number.
 6. Estimate the loss of the accident, and rescue the injured and property.
 7. Information about witnesses, names, addresses and contact numbers of witnesses.
 8. Signature and seal of the autograph or proxy. Write the date of the accident.
 The materials shall be written with blue and black pens, and the writing shall be neat and clear.
