Yesterday, in Yongkang Orthopaedic Hospital, the reporter of Qianjiang Evening News saw several shocking photos.

The little girl is 5 years old and her name is Xiao Bang. She and her parents opened a sawmill on the first floor of their residence. On the afternoon of December 16, Xiao Bang ran into the factory to play. His right wrist was accidentally cut off by the moving machine, and the whole palm fell off.

Fortunately, they were sent to the hospital in time. Eight hand surgeons who had already left work were called back halfway and worked overtime to perform the operation which took six hours.

At present, Xiao Bang's broken hands have been connected and the situation is stable. The doctor said that it would be OK to restore normal function in the future, but the strength would be weak.

In the evening, a man rushed into the emergency room

Shout: "Save my daughter"

At 5:00 p.m. on December 16, when the sky was getting dark, Yongkang Orthopaedic Hospital slowly quieted down. Suddenly, a sudden cry for help broke the silence.

"Doctor, doctor, please save my daughter." A young man wearing a hat rushed into the emergency room with a little girl in his arms.

The girl's right hand was broken at the wrist, the whole palm fell off, and the wound was still dripping blood. Her jeans were full of blood. Maybe it hurts too much. The girl seems very quiet at this time. She opened her big eyes and looked at the crowd, neither crying nor making any noise.

Following the man, there were three others. Among them, a young woman held a cloth bag in her hand, which was the girl's broken palm. This severed palm has no blood color, and its color has changed to banana yellow. The place where the broken palm connects to the wrist looks like blood and flesh.

The girl's name is Xiaobang. She is 5 years old and she is in the middle class of kindergarten. The man holding her is his father, Mr. Liu, and the one holding the cloth bag is his mother, Ms. Xu.

Mr. Liu works in a wood factory in Dushantou Village, Zhiying Town, Yongkang City, and Ms. Xu works in a nearby enterprise. A family of three lives on the fourth floor of a residential building. Ms. Xu told the reporter of Qianjiang Evening News that the accident occurred at about 4:30 p.m. on December 16.

"When I picked up my daughter from the kindergarten, I went upstairs to prepare dinner." Ms. Xu said sadly with a regretful face, "My daughter and son stayed downstairs to play in the open space, and there were several other children."

According to Ms. Xu, at that time, Xiao Bang was afraid when he saw a madman walking along the road, so he hid in the sawmill on the first floor.

When the machine was running, a worker was sawing wood, and the narrow aisle was blocked. Xiao Bang wanted to go there, so he walked close to the workbench and accidentally put his hand under the saw.

It hurts to think about that scene. When Ms. Xu heard the news, she ran downstairs and found her daughter's right hand was gone. The worker picked up the broken palm that fell on the ground and put it on the workbench.

"At that time, I was stunned," Ms. Xu said.

Were called back halfway from work

After the accident, the owner of the timber factory drove his minivan to the hospital with three members of Xiaobang's family. "If you wait for 120, I'm afraid it's too late," said his father, Mr. Liu.

The city road at 5 p.m. is the evening peak. The minivan went all the way and stopped all the way. Xiao Bang cried in pain. The adults were even more anxious.

At about 5:15, the group finally arrived at Yongkang Orthopaedic Hospital. Seeing Xiao Bang's wound, the young doctor on duty was also shocked. Because of the urgency, everyone didn't bring much money, and the hospital opened a green channel. At this time, time is life. The earlier the operation is performed, the more likely Xiao Bang's hand will be taken back.

Xiao Bang was pushed into the operating room at 5:30 p.m. At this time, most doctors are leaving work. Sun Yizheng, director of hand surgery in the second ward, was about to start the car to go home, while Xu Wang, director of the Anesthesiology Department, hurried to eat something to watch a ball game.

After receiving the call from the emergency room, the two rushed to the operating room at the first time. Hearing that the patient was in an emergency, Sun Yi called all the other seven doctors in the department while he was on his way.

This is a very delicate operation

All 30 tendons and veins on the wrist should be sewn

Sun Yi knows that replantation of a severed hand is a race against time, and time is the key.

The doctors were divided into two groups. One group was responsible for cleaning the stump (amputated palm) and near amputated (arm wound). They had to sort out and mark the muscles and veins. The other 4 people began to prepare blood, operating bed, and communicate with the patient's family before operation.

At 7 p.m., with the assistance of two nurses and two anesthesiologists, the operation officially began.

During the operation, 8 hand surgeons were also divided into groups. One group was operated on, and the other group was on standby.

Lei Ming, a hand surgeon, said, "Even if we don't participate in the operation, we must stay in the operating room all the time. If someone can't stand it, we can immediately push it up."

Sun Yi said that although he was sent to the hospital in time, Xiao Bang's replantation of the amputated palm was also full of risks.

The biggest challenge is to connect all the severed muscles.

"Generally speaking, controlling the flexion and extension of a finger requires 3 tendons, 2 tendons to control the flexion and 1 tendon to control the extension." So, if there are 5 fingers in a palm, there are 15 tendons. Plus 2 arteries, 4 nerves, 5 or 6 veins, and 4 tendons on both sides of the wrist, the total number of wrist muscles and veins is about 30.

"If you want to fully restore the function of the patient's fingers, you must connect these muscles one by one." Sun Yi said that because the children's muscles and veins are very small, the surgery is carried out under the microscope, "the suture used is thinner than the hair."

As a knife wielder, Sun Yi is undoubtedly the most tired of all. Long term operation is physically exhausting, especially when facing the microscope, the eyes are more easily tired. Fortunately, every hour of this operation, it needs to stop for 10 minutes to supply blood to the amputated palm. During this time, Sun Yi quickly wiped his face with cold water and massaged his eyes.

"One to three tendons can be connected in one hour," Dr. Sun said.

The girl's current situation is stable

The doctor said that normal function can be restored

The operation lasted until 1 am the next day and took six hours to complete. At this time, nearly 9 hours had passed since the little girl's palm was cut off.

"According to the routine, it will take at least 10 hours to perform such an operation. Our entire department will go on the battlefield, reducing the time to the minimum," said Sun Yi. Because the patient is young, has low immunity, low blood volume, and poor tolerance, the risk is much higher than that of adults.

According to his understanding, replantation of severed hands of 5-year-old children is rare in the country.

Yesterday, the reporter of Qianjiang Evening News learned that Xiao Bang's broken hand has now recovered its color and the situation is stable. However, they are not completely out of danger and need to be closely monitored.

"Within a week after the operation, tissue necrosis or toxin concentration may occur, not only affecting the palm, but even endangering the whole body. After this dangerous period, the operation can be said to be successful." However, Sun Yi said that if Xiao Bang's operation is successful and after rehabilitation training, daily life will be OK.

Previously, he had a hand replantation operation, and the patient's palm recovered to 90% of its normal function. "Except for the strength, other functions are the same."

Some common sense of amputation

How much do you know

1. Injury mode of severed fingers and limbs. Cutting injury is the easiest to replant, while crushing injury and strangulation injury are difficult to replant. Because the latter two are seriously injured and damage a large number of tissues, the ions and inflammatory factors released by these injured tissues will damage the remaining normal tissues, causing a large number of tissue necrosis and irreparable damage.

2. Time and storage method. In general, the less broken tissue, the longer the storage time. Simple severed fingers can be preserved for 8-12 hours if well preserved, while severed limbs can only be preserved for 4-6 hours or even shorter. As for the preservation method, there are two principles: dry and cold. The simplest way is to wrap the severed limb with an impermeable cloth or plastic bag, and apply ice on it.
